Childhood Cancer Awareness Month is an important time to recognize the children and families affected by cancer while also helping people better understand what childhood cancer care can involve.
For many families, medical imaging becomes part of that journey. Depending on the type of cancer and the questions a care team needs to answer, imaging may be used to investigate abnormal findings, determine the extent of disease, plan treatment, or monitor how a child is responding.
This is where pediatric radiology plays an important role. Children are not simply smaller versions of adults, and their imaging needs can differ in meaningful ways. How Is Childhood Cancer Diagnosed?
There is no single test that diagnoses every childhood cancer.

The FDA recommends using the lowest radiation dose necessary to obtain images that are adequate for diagnosis or treatment guidance in children.
MRI
MRI uses magnetic fields and radio waves rather than ionizing radiation. It can provide highly detailed views of the brain, spinal cord, bones, bone marrow, muscles, joints, and other soft tissues. Pediatric MRI may be used to detect certain tumors or monitor response to cancer treatment.
Nuclear Medicine and PET Imaging
Some children may undergo nuclear medicine studies, including PET-based imaging, depending on their diagnosis. These exams use small amounts of radioactive material to provide information about activity within tissues. They can help physicians evaluate certain cancers and, in some circumstances, assess response to treatment.
How Imaging Helps Determine the Extent of Disease
Once cancer has been diagnosed, physicians often need to understand where it is located and whether it has affected other parts of the body.
This process is commonly referred to as staging, although the exact staging system varies by cancer type. Imaging may help physicians assess:
- The size and location of a tumor
- Nearby lymph nodes
- Involvement of surrounding tissues
- Whether disease is present elsewhere in the body
- Important anatomical relationships that may affect treatment

For a child who needs surgery, scans can provide information about a tumor’s relationship to nearby organs, blood vessels, bones, or nerves.
Imaging may also be used when planning radiation therapy or certain image-guided procedures.
The exact role varies significantly from one child to another. A child with leukemia, for example, may have very different imaging needs from a child with a brain tumor or bone cancer.
That is one reason personalized imaging decisions are so important in pediatric oncology.
How Doctors Monitor Response to Treatment
Imaging may also be repeated at specific points during treatment.
By comparing newer images with earlier studies, physicians can evaluate whether a tumor has changed in size or appearance and determine whether there are other meaningful changes.
This information may help the oncology team assess how treatment is progressing.
However, imaging is rarely interpreted in isolation. Physicians may also consider blood tests, pathology findings, symptoms, physical examination results, and other measures when evaluating treatment response.
What Happens After Treatment?
For some childhood cancer survivors, imaging may continue after active treatment ends.
The type and frequency of follow-up depend on factors such as the original diagnosis, the treatment received, the child’s current health, and the risk of recurrence or treatment-related complications.
The National Cancer Institute recommends maintaining detailed records of childhood cancer treatment and follow-up, including the types and dates of imaging examinations performed.
Not every survivor requires the same type or amount of imaging. Follow-up plans are individualized to avoid unnecessary testing while still addressing appropriate medical concerns.
Why Pediatric Radiology Is Different
Children’s bodies change significantly as they grow, and imaging techniques often need to change with them.
For exams that use ionizing radiation, such as CT or X-ray, equipment settings can be adjusted based on factors including a child’s size and the part of the body being examined. The goal is to obtain the information physicians need without using more radiation than necessary.
Pediatric imaging also involves practical considerations that may not be as significant for adults.
Younger children may need additional reassurance before an exam. Some imaging studies require children to remain very still, which can be challenging depending on their age and the length of the procedure. Parents may also have questions about contrast material, preparation, radiation exposure, or what their child will experience.
