Each September, Pulmonary Fibrosis Awareness Month brings attention to a group of lung diseases that can make breathing increasingly difficult. Pulmonary fibrosis occurs when tissue within the lungs becomes thickened and scarred, making it harder for oxygen to move from the lungs into the bloodstream.
For someone experiencing unexplained shortness of breath or a persistent dry cough, determining the cause may require several different types of evaluation. Medical history, a physical examination, lung function testing, and imaging can all provide different pieces of the picture.

What Is Pulmonary Fibrosis?
Pulmonary fibrosis is not a single disease. The term describes scarring, or fibrosis, within the lungs that can develop for different reasons.
Some forms are associated with autoimmune conditions, environmental or occupational exposures, certain medications, or other diseases. In other cases, physicians cannot identify a specific cause. This is known as idiopathic pulmonary fibrosis, or IPF.
As scar tissue develops, the lungs can become stiffer and less able to expand normally. This can interfere with the body’s ability to get enough oxygen.
What Symptoms Can Pulmonary Fibrosis Cause?
Symptoms often develop gradually, which can make pulmonary fibrosis difficult to recognize at first. Possible symptoms include:
Shortness of breath, particularly during activity
- A persistent dry cough
- Fatigue or weakness
- Unexplained weight loss
- Aching muscles or joints
These symptoms can occur with many other health conditions, so having them does not mean you have pulmonary fibrosis. Persistent or worsening breathing changes should be discussed with a healthcare provider who can determine what type of evaluation is appropriate.
Why Is Imaging Used When Pulmonary Fibrosis Is Suspected?
A physical examination can provide important clues about lung health, but physicians cannot directly see changes deep within the lungs without imaging.
A chest X-ray may be one of the first imaging tests performed. It can sometimes reveal signs of lung damage or scarring, but its level of detail is limited.
When more information is needed, physicians may recommend CT imaging.
How Can a CT Scan Help Evaluate Pulmonary Fibrosis?
CT imaging creates detailed cross-sectional views of the chest. Instead of showing the lungs as a single overlapping image, CT allows radiologists to examine thin sections of lung tissue.
This added detail can help physicians evaluate:
- The presence and distribution of lung scarring
- Changes within the tissue surrounding the lung’s air sacs
- Areas of inflammation
- Patterns associated with different forms of interstitial lung disease
- The extent of abnormalities within the lungs

What Is a High-Resolution CT Scan?
When pulmonary fibrosis or another interstitial lung disease is suspected, physicians may order a high-resolution CT, often referred to as HRCT.
HRCT uses specialized CT techniques to produce very detailed images of lung tissue. These images allow radiologists to examine subtle patterns that may not be visible on a standard chest X-ray.
For example, physicians evaluating suspected idiopathic pulmonary fibrosis may look for a specific pattern of abnormalities within the lungs. Certain CT findings can provide important information about whether IPF or another type of interstitial lung disease may be present.
Can a CT Scan Diagnose Pulmonary Fibrosis?
A CT scan provides valuable diagnostic information, but pulmonary fibrosis is not diagnosed based on imaging alone in every case.
Your healthcare team may consider several sources of information, including:
- Your symptoms and medical history
- Occupational or environmental exposures
- Medications
- Family history
- Lung function testing
- Blood tests
- CT findings
Sometimes additional procedures are necessary to better understand the cause of lung changes.
This is an important distinction: a CT scan helps physicians see and characterize abnormalities within the lungs, while the final diagnosis depends on the complete clinical picture.
Why the Pattern of Lung Changes Matters
Pulmonary fibrosis can appear differently depending on its underlying cause.
Radiologists do more than simply determine whether scarring is present. They also examine where abnormalities occur and how they are distributed throughout the lungs.
Certain patterns may point physicians toward particular types of interstitial lung disease or help distinguish one condition from another. This detailed assessment is one reason high-resolution CT has become such an important part of evaluating suspected pulmonary fibrosis.
What Happens During a Chest CT?
A chest CT is a noninvasive imaging exam.
During the scan:
- You lie on a table that moves through the CT scanner.
- The scanner captures images of your chest from multiple angles.
- You may be asked to hold your breath briefly to reduce movement.
- A radiologist reviews the completed images.
The imaging portion of the exam is generally quick. Your preparation will depend on the specific type of CT your physician orders, so always follow the instructions provided before your appointment.
What Happens If the CT Shows Lung Scarring?
Finding lung scarring on a CT does not automatically establish its cause.
Your physician may use the imaging findings together with other test results to determine whether you need additional evaluation, such as pulmonary function testing, laboratory studies, consultation with a pulmonologist, or other diagnostic procedures.
The goal is to understand not only whether fibrosis is present, but also what may be causing it and how it is affecting lung function.
